A coronial inquest into the circumstances surrounding a woman's death at Alice Springs Hospital has heard evidence suggesting systemic procedural failures. During the final day of hearings yesterday, the coroner heard the 33-year-old patient died after receiving an extremely high dose of sedatives by medical staff.
